As a Sapphire Health Services Community, Gateway Care and Retirement Center provides compassionate and personalized skilled nursing care 24 hours a day. This 59 bed facility specializes in Skilled Nursing and Rehabilitation services to a diverse population.

The rehab team is led by the Rehab Director, who is passionate about providing what the patients need for quality outcomes in addition to advocating for patients’ well being. When Consonus began providing services here, the administration immediately showed support in expanding the gym and creating a strong rehab presence. The rehab department is valued and has a prominent seat at the table

The expanded gym allows for more patient to patient interaction, which is motivating when striving for optimal outcomes. The therapists are embracing ACP modalities in addition to Consonus Clinical programs.
